Based in our Bunbury office including travel to the Collie area, the Mental Health Program Facilitator will promote and enable access to NDIA services and increase social participation and build individual capacity for daily living through appropriate referrals to health and community services, whilst ensuring all work is aligned and compliant to all legislative and contractual requirements and in accordance with Forrest Personnel Policies and Procedures.

Job Description

You will:

  • Promote and enable persons living in the community to confidently access the services available to them.
  • Support people with a severe mental illness and associated psychosocial functional impairment to lodge access / access requests through the National Disability Insurance Scheme (NDIS).
  • Provide individual and flexible support to, and advocate for participants as they navigate the NDIA and health care system.
  • Have an excellent knowledge of local community services and mental health supports that are available in the local region and establish and maintain respectful, collaborative and professional relationships and boundaries with all stakeholders.
  • Work with existing case managers, both internally at Forrest Personnel as well as partner services to provide Psychosocial/Mental Health supports.

Desired Skills and Experience

To be considered for this role you will need:

  • Tertiary qualifications in Counselling/Mental Health and/or other relevant qualifications.
  • Previous experience in a relevant human services field.
  • Sound understanding of the provision of Mental Health Services in WA and of Community Health Services in a broader sense.
  • Understanding of Recovery and demonstrated ways of working with individuals who experience mental distress to promote a high expectation of Recovery and facilitate change.
  • Strongly demonstrated ability to operate within Organisational guidelines, procedures and policies.
  • Previous work with community members in an outreach setting (desirable).
  • Highly developed oral and written communication and interpersonal skills.
  • Advanced computer skills, with experience in MS Office applications.

We’re also looking for a candidate with current

  • Current Driver’s Licence
  • Current Working with Children Check
  • National Police Clearance (within the last 3 months)
